Patient Referral
We welcome all patient referrals and provide a Free Hearing Test and Comprehensive Evaluation. A complete report will be sent to your office following the appointment for the record.
Over the years, physicians have referred hundreds of patients to Whisper Audiology for hearing tests, hearing aid evaluations, tinnitus management, and hyperacusis consultations. Fill out the “Referral Form" below and in the comments please indicate the pertinent patient information.

In 2018, Jasmin, our head Audiologist opened her first clinic in Stoney Creek in hopes to provide individualized service to every patient with the best patient care as her core philosophy. We work alongside many other healthcare providers in order to provide the best hearing health care service. We deal with all major hearing aid manufacturers, which means patients will always be recommended what is best or their specific needs. Our goal is to improve the quality of life for our patients by providing exceptional and affordable hearing healthcare in a compassionate environment. Our mandate includes the prevention, evaluation, and treatment of hearing loss for all ages. We take pride in putting patient hearing health care above all else.
